2. Photo & Media Release Permission
I grant permission for photos and/or videos taken during the conference to be used for:
Church website
Social media platforms
Promotional materials
Printed publications
Internal ministry communications

I understand that:
Images will be used for ministry purposes only.
No compensation will be provided.
I may request removal of a specific photo by contacting the church office.

3. Acknowledgment and Agreement
By entering my name below, I acknowledge that I have read and understood this entire document and agree to all terms and conditions stated above.

I agree to follow and adhere to the rules, guidelines, and directions set forth by the church, and to participate in the conference accordingly.

I understand that, in the event of an emergency, the leader(s) will attempt to contact me or the emergency contact person listed below. If neither a parent nor the emergency contact can be reached, I authorize the leader(s) to take appropriate and reasonable action to care for the child, including transportation to and treatment at a hospital emergency room if deemed necessary at the discretion of the leader(s) or the church.

I hereby release and agree to hold harmless the church and its designated leaders, staff members, teachers, teaching assistants, and volunteers—whether paid or unpaid—from any liability for damages, illnesses, or injuries that may arise before, during, or after participation in the event.
