I, as the legal guardian of the aforementioned student, grant permission for my child to actively participate in events organized by Korean Central Presbyterian Church's Agape Youth Ministry. 나는 위에 언급된 학생의 법적 보호자로서, 나의 자녀가 KCPC 아가페 청소년 사역에서 주최하는 행사에 적극적으로 참여할 수 있도록 허락합니다.
Participation Agreement / 참여 동의:
I understand and agree that my child will adhere to the rules and guidance provided by the event leaders, including pastors, teachers, and appointed staff members. 나는 내 자녀가 행사 리더, 목사, 교사 및 지정된 스태프들의 규칙과 지도를 따를 것에 동의하고 이해합니다.
Transportation Authorization / 교통 수단 권한 부여:
I authorize my child to utilize church-designated transportation, operated by drivers appointed by the church or event leaders, for the duration of the event. I acknowledge that the church assumes no responsibility or obligation if my child chooses not to use the designated transportation for participation in the event. 나는 내 자녀가 교회나 행사 리더에 의해 지정된 운전사가 운전하는 교회 지정 교통 수단을 사용할 수 있도록 허락합니다. 교회는 내 자녀가 행사 참여를 위해 지정된 교통 수단을 사용하지 않을 경우에 대한 책임이나 의무가 없다는 것을 알고 있습니다.
Emergency Contact and Care / 비상 연락 및 보호:
In the event of an emergency, I acknowledge that the event leaders will make every effort to contact either myself or the designated emergency contact person listed below. If neither parent nor the emergency contact person can be reached, I grant the leaders permission to take appropriate and reasonable actions to ensure the well-being of my child. This may include seeking medical attention, such as a visit to the hospital emergency room, if deemed reasonably necessary at the discretion of the event leaders or the church. For non-emergency situations, I understand that I am responsible for picking up my child from the event location if they are mildly ill or need to leave for personal reasons. 비상 상황 발생 시, 나는 행사 리더가 나 혹은 아래에 명시된 비상 연락인에게 연락할 것을 인지하고 동의합니다. 부모나 비상 연락인과 연락할 수 없는 경우, 리더가 내 자녀의 안전을 보장하기 위해 적절하고 합리적인 조치를 취할 수 있도록 허락합니다. 교회나 리더의 재량이나 교회에서 합리적으로 필요하다고 여길 경우 응급실 방문과 같은 의료 조치도 포함될 수 있습니다. 비상 상황이 아닌 경우, 자녀가 가볍게 아프거나 개인적인 이유로 이탈해야 할 경우 자녀를 픽업해야 한다는 것을 이해합니다.

I give permission for the Child to ride in the vehicle designated by the Church or leader(s), operated by a driver designated by the Church or leader(s). I am neither obligated nor required to have the Child ride in the vehicle designated by the Church or leader(s) in order for the Child to participate or attend the Event.나는 자녀가 교회나 리더가 지정한 교통수단와 교회와 리더가 지정한 운전사가 운전하는 교통수단을 이용할 수 있도록 허락합니다. 나는 자녀가 행사에 참여하거나 참석하기 위해 교회나 리더가 지정한 교통수단을 이용하는 것에 대해 교회가 어떠한 책임이나 의무가 없음을 허락합니다.
Liability Release / 책임 면제:
I hereby release and agree to hold harmless the Korean Central Presbyterian Church, its designated leaders, staff, teachers, teaching assistants, and volunteers (whether paid or unpaid) from any liability for damages or injuries incurred by my child arising from, during, and after the church-related event. 나는 KCPC, 지정된 리더, 스태프, 교사, 보조 교사 및 자원봉사자(유급이나 무급 상관없이)에게 발생한 피해나 상해에 대한 책임을 묻지 않을 것을 동의합니다. 이는 행사 전, 중, 후에 발생하는 것을 포함합니다.
Video/Photo Policy 사진/영상:
I hereby consent the photographing, recording, reproduction of any other manner (including use of video and audiotapes) of likeness, voice and/or activities of the participant and further authorize KCPC to make unlimited use of recordings for educational and worship purposes. I do hereby release and hold harmless KCPC, its employees and volunteers from any claims. 본인은 자녀의 사진 촬영 또는 비디오 녹음에 동의하며 KCPC가 교육 및 예배 목적으로 녹음을 무제한으로 사용할 수 있도록 허락합니다. 본인은 이에 대해 KCPC, 직원 및 자원 봉사자들에 대한 어떠한 손해 배상을 포기합니다.
Refund Policy:
I also understand that in case I need to cancel my child's registration, I will not be refunded unless I cancel by 5/5/2024 (partial refund 1/3). NO REFUND will be given after this date. 또한 만약 등록을 5/5/2024일까지 취소할 경우에만 등록비 일부(1/3)를 받을 수 있으며, 그 이후에는 등록비 환불이 되지 않음을 인정합니다.
